An inspection by Gallatin fire inspectors uncovered 11 fire code violations, one of which involve d the buildup of combustible dust.  Similar conditions were cited by the US Chemical Safety Board following the deaths of five workers in 2011.

Senior Fire Inspector Mark Parrish described the violations as semi-serious, "not minor" but also "not major."
